To summarize, I think if you host the picture somewhere else (what used to be the only real option) and wrap the link to the picture in [I M G] [/I M G] tags (without those extra spaces), then whatever size the image is on the picture host, it will be displayed. If you upload the image and store it on this server, then it seems to shrink it and display a thumbnail.
To better describe the options, both of which can be done using the "insert picture" button, here are your two options.
In this case, I click the insert image button, choose "From URL" and UNCHECK "Retrieve remote file and reference locally." This means a copy of the picture isn't stored on the server and it is inserted using the old IMG tags:

Now, you could also choose "from computer" and it would also upload a copy of the picture to the server and display a thumbnail.
To be honest, I just now played with the settings to see if you could get it to use the old style IMG tags to pull from the host. Now that I've looked at it, it makes sense, but the last few pictures I've posted, I either manually used the IMG tags or let it attach with the thumbnail. I hadn't really paid attention to that option before.
