In what month and year and how the name was chosen and by whom suggested the name?

I can only answer the third part of that question ,a very good question at that I'll bet Topscribe can answer that.
I got this out of the Mile High Magazine pg 98, and I found it to be interesting knowing the birth of the football Denver BRONCOS.
The name "Denver BRONCOS" was chosen from 500 entries in a name the team contest in Jan of 1960. The name was suggested by Ward M. Vining of Lakewood. But the football team isn't the first Denver BRONCOS. Denver's 1921 entry in the Midwest Baseball League was also called the BRONCOS.

Without looking it up, it was quite a whirlwind deal, the founding of the
franchise. Upon the meeting in 1959 of Lamar Hunt and Bob Howsam, the
original owner, the AFL, and the Denver Broncos, were founded. The Denver
Broncos franchise came into existence shortly thereafter . . . for the price
of $25,000, believe it or not.
A fan contest was then held in 1960 to determine the mascot, which a
young lady won, as I remember, with the name "Broncos." (Boy I hated that
name at the time. I had entered with the name "Diamondbacks,"
and my ego couldn't take that being beaten out by "Broncos." It grew on
me, though. ) Off the top, that's all I can answer right now. I might
remember more later.
Oh, and the second part of your question, the original Denver Broncos was
a minor league baseball team in the 1920s, I believe.
On the side, the city of Denver was not always a Colorado city. It was
founded in 1858, named after James Denver, the governor of Kansas
Territory at the time. (I don't know why I added this. It just occurred to
me, that's all, I guess. )
