    Really? I do feel like I could lift more, but I'm also pushing that weight with everything I got. I'm normally doing 4 warm-up sets before I go to my full weight. For example on Squat I did:

    45.0x5
    95.0x5
    95.0x5
    135.0x3
    175.0x5
    175.0x5
    175.0x5
    175.0x5
    175.0x5

    I'm a little hesitant to diverge much from the 5x5 formula until I've completed 12 weeks

    If banging the rust off is more uncomfortable than the last two work sets, then yes.

    You could also try doing your last warmup set with a bit more weight than your working sets for just a handful of reps. Some people recommend doing that as well just so your muscles know its time to work. Also, doing a few sets of five is probably not an adequate warmup. Try doing a couple of warmup sets of 12-15 with 85...get some blood in the muscles.

    There should be plenty of blood in the muscles at that point since I either spend 15 minutes on the eliptical, or run a mile and a half before I lift.

    Try skipping all that before the workout and just do a five minute warm up then roll in to weights

    Skipping 15 minutes of cardio on leg day isn't going to hurt your weight loss efforts if your diet is on point. You might even want to try pre-exhausting your legs a bit before hitting the squat rack. You should try a couple of different things because by that fourth and fifth set you should be struggling on the last rep or two.
